It's time for you to enroll for Dependent Care and Medical Reimbursement
programs in CPCC's Flexible Benefits Plan for January 1, 2005, to
December 31, 2005. You will receive your enrollment forms by Wednesday,
November 24, 2004. The enrollment forms are due back in HR (Benefits)
December 7, 2004.

 

The Flexible Benefits Plan allows you to open a pre-authorized account
with Interactive Medical Systems to pay for eligible expenses prior to
federal, state, and FICA taxes being deducted. The attractive advantage
to a Flexible Benefit Plan is the tax break:  Flexible Spending Account
for Non-Covered Health Care Expense (Limit $3,000), Flexible Spending
Account for Dependent Day Care Expenses (Limit $5,000).  

 

According to Federal Law, all employees are required to sign a form
stating that CPCC offered them the benefits of Section 125. This does
not obligate you (the employee) to participate. Those choosing to
participate may use the attached worksheet to estimate your
un-reimbursed expenses for the coming year. You should bring this form
with you to the enrollment workshops listed below.

 

Faculty and staff employed between January 1, 2004, and present should
make special note to attend the programs since this will be your first
enrollment opportunity. Interactive Medical System Associates will be on
Levine Campus and Central Campus to answer questions and to assist in
enrolling faculty and staff.

Current employees participating in Dependent Care and Medical
Reimbursement program are reminded that this is the time to change
allotments or terminate participation.  Please plan to review your
program with the representatives.  All enrollment forms are due in HR
(Benefits Office) by Tuesday, December 7, 2004.
