The circumstances and time periods for coverage are outlined in the document entitled "Continuing Coverage at Carolinas Medical Center and affiliated hospitals After September 27, 2003." http://www.cpcc.edu/humanresources/forms/default.htm Please note that if members or dependents wish to continue coverage, they must complete the Continuity of Care Form and return it to the Claims Processing Contractor at the address noted on the form. Members who choose services at Carolinas Medical Center or its affiliated hospitals listed above and who are not eligible for continuing coverage, should be aware that they will be responsible for an additional 20% coinsurance up to $5,000 per member ($15,000 per family) and Carolinas Medical Center may bill members for any difference between the hospital's charge and the allowable amount paid by the Plan. This applies to all Plan members who utilize inpatient or outpatient services at Carolinas Medical Center Hospitals, except for those special circumstances involving continuity of care, as indicated above. The Continuity of Care form and a sheet explaining continuity of care are found on the HR webpage. Click the FORMS button and look under the Benefits category to find copies of these documents. In order to be able to grade your classes online, however, it is critical that all instructors, full-time and part-time, have network login ID's and passwords in advance. Those who do not have network access should request it now. To do so, go to the CPCC homepage, click on "CPCC Employees," click on the "Instruction" link at the left and then click on Course Administration Webtools. There you will find a "request an account now" link. IMPORTANT: Request access to the "College Network" only. You do not need mainframe access to enter grades online. If you have a login and password, but don't know if it is active, follow the steps above and attempt to log in at the Course Administration Webtools site. If you get to the "Enter Grades" link, your password is active. If not, then go to the "request an account" link and complete the form. Printable instructions for web grading as well as a training walk-through are available after logging into the Course Administration Webtools site and clicking on "Enter Grades." In addition, assistance with training or entry of grades is available at every campus through the Offices of Admissions, Registration and Student Records. Check with the Campus Registrar at your campus. John Chapman, better known as Johnny Appleseed, was born September 27, 1774. He had a great love for all humanity and all living things and spent much of his life distributing apple trees to the settlers of the American frontier. He loved to tell stories of his adventures to the children he met as he traveled. Johnny Appleseed believed that we must try to live simply and in harmony with the natural world. Please join us at Levine Campus as we celebrate the birthday of one of America's great frontier heroes. The festivities begin with the fabulous Burford Brothers playing some foot-tapping, hand-clapping, down-home tunes on fiddle and banjo. We'll also have storytelling, face-painting, coloring, games, and this year's fresh crop of apples (which just came in time for Johnny's birthday). Everything is free for our CPCC apple-loving family and their families, including applelicious waffles for dinner and apple pie for dessert. We hope you help spread the word to the students and come join us this Friday between 5:00 and 7:00 pm. --Barry Gilmore -------------- This message did not originate from the Office of Community Relations. All remaining hours should then be coded as COMP. Any time sheets that do not comply with the compensatory guidelines (the guidelines were distributed at the time sheet trainings) will not be entered. Please review the attached guidelines on the HR website. http://www.cpcc.edu/humanresources/timesheetguidelines.doc If you have questions, please complete a Leave Discrepancy Form and/or contact me at ext. 6314 for an appointment to review your concern. > The code FSIC is applicable to faculty employees with a 10-month contract that is paid in 10 months. All others should use the SSIC code. > Please add all time taken for the month and enter the final total in the code box located at the bottom of your time sheet. > When submitting time sheets for your area, please code and total the hours accurately to avoid incorrect data being entered. Information is entered according to what has been recorded on the time sheet for that month. > I have now set aside Fridays as a day to research leave accounting concerns. This is to ensure that all discrepancies will receive a resolution. Thank in advance for your cooperation. --Schnell M.
