The Voluntary Election of Faculty Contract Duration/Salary Payments and Benefits Deduction Form is available online for all new and existing full-time faculty to complete to select a  method of monthly salary payments for the fiscal year 2003-2004.

This form is used to determine the number of monthly salary payments and the method of payment for optional benefit coverage.  Each full-time faculty employee will voluntary elect an option.  The selection option will be in effect annually.  

Faculty should notify the Human Resources Analyst of any changes in options. The options contained in the form will be subject to all pertinent policies and procedures outlined in the Policies and Procedures Manual, and subject to compliance with any legislative change. 

Employees electing contract payments of less that 12 months must make arrangements with the Benefits Division to pay for optional benefits during the summer months.

Please complete this form as soon as possible and send back to the Human Resources Department, Attention Cheryl Marshall, on or before August 11, 2003.  

This form is available online. Go the Human Resources website, click on Forms.  Under the Compensation heading, click on Voluntary Election of Faculty Contract Duration/Salary Payments and Benefits Deduction Form.

All newly hired full-time faculty must complete this form.  Existing full-time faculty only need to complete the form if changing monthly payment option.
